Proof of liability insurance required

We are getting ready to have a Chapter Event in September and need to reserve a place in the county park. The County is requiring proof of insurance. Anyone know how to obtain the proof of insurance?

A certificate of insurance will be issued whether Thrivent is the sole sponsor or the co-sponsor of the activity. We would need to gather the following information:
Date of event
Chapter name and number
Email address of the chapter leader
Description of the event
Name of facility where event will be held
Street address, city and state where the event will be held

Please email the information to FRATERNAL@THRIVENT.COM. Please identify the name of your chapter. After you have gathered this information, we will refer it on to the liability person in our office to respond via email.
We will forward to the chapter leader the certificate of insurance by email.
